Immunization Requirements
K - 8 Students
- Hepatitis B (series of three doses)
- Diphtheria, Tetanus and Pertussis (DTP)
- Tdap - required for ages 11-12
- Polio
- Measles, Mumps and Rubella (MMR) (two doses)
- Varicella (Chickenpox) vaccine*
- Meningococcal (MCV4) - required for ages 11-12
Preschool Students
- Hepatitis B
- Diphtheria, Tetanus and Pertussis (DTP)
- Polio
- Haemophilus Influenzae b (HIB) series
- Measles, Mumps and Rubella (MMR) vaccine
