Checklist for the First Day of School

Whether your student is starting their very first day of school or heading into their final year of middle school, there are a few key things every family should keep in mind when preparing for a new school year. Review our checklist of things to think about to help you and your student start the 2025–2026 school year with confidence and ease.

Immunization Requirements

K - 8 Students

  • Hepatitis B (series of three doses)
  • Diphtheria, Tetanus and Pertussis (DTP)
  • Tdap - required for ages 11-12 
  • Polio
  • Measles, Mumps and Rubella (MMR) (two doses)
  • Varicella (Chickenpox) vaccine*
  • Meningococcal (MCV4) - required for ages 11-12

Preschool Students

  • Hepatitis B
  • Diphtheria, Tetanus and Pertussis (DTP)
  • Polio
  • Haemophilus Influenzae b (HIB) series
  • Measles, Mumps and Rubella (MMR) vaccine
  • Hepatitis A vaccine**
  • Varicella (Chickenpox) vaccine*

*Students entering childcare, preschool, kindergarten, and grades 1 through 8 will be required to have proof of receiving the Chickenpox vaccination (Varicella) OR a history of having had the Chickenpox disease.

**Children ages 1 through 5 years old who attend a licensed day care facility or Head Start in Maricopa County must also receive Hepatitis A vaccine.
