The Transportation Department will give both the "Student Information Card" and "Alternate Adult to Receive Student" forms to parents/guardians the first week of school. Parents/guardians are asked to complete the forms and return them to your child's bus driver as soon as possible.
The Student Information Card provides the driver and assistant with important information to ensure your child's safety. The information provided on this form is kept confidential and only given on a need-to-know basis.
The Alternate Adult Form enables the driver and assistant to recognize who can accept the student should the parent/guardian not be there to accept. The Authorized Alternate Adult must provide identification to the driver before release of your student. In instances where a parent/guardian needs an alternate adult to receive a student and they do not have a form readily available, a letter must be sent to the Transportation Department authorizing the release of your student prior to drop-off. This letter is maintained in the Transportation Department and is used in lieu of the form or until the form is completed by the parent/guardian. Without the letter or Alternate Adult Form, your student will not be released, but taken back to school.
For students who begin special needs transportation after the start of the school year, the Alternate Adult to Receive Student form will be completed during the IEP process or will be given to the parent/guardian by your child's bus driver during pickup or drop off after transportation begins. They can then complete the form and return to the driver.
Both the Student Information Card and Alternate Adult to Receive form should be updated annually.
